For Trust Officers
Our Care Managers can assist Trust Officers by:
• Delivering expert knowledge of all areas impacting the senior or person with disabilities from medical issues, health care systems, in-home care to behind the scenes knowledge of the various care communities
• Promoting client’s independence and quality of life while focusing on appropriate level of care for the individual and advocating for the client at all care levels
• Assisting long distance trust officer in managing care for clients
• Helping preserve assets by ensuring client remains in most appropriate care setting for as long as possible
• Taking on medical and direct support tasks freeing the trust officer to focus on financial management
• Protecting the fiduciary by ensuring the money spent is done so for the benefit of the recipient
• Providing personal compassionate service—focusing on the individual’s values and needs while maintaining professional oversight of care
• Arranging for and monitors in-home health care and other services requested by client to meet care needs
• Providing resources and options for clients and families regarding care decisions including next steps/transfers to different levels of care
• Assisting family members who live out of the area to facilitate long-distance caring for a loved one
